YSC Student Referral Form for School Employees

What is the Meyzeek Middle School Youth Services Center?

The Meyzeek Youth Services Center (YSC) works with parents, community representatives, and school staff to create a positive, nurturing, learning environment that will empower youth to become successful students and productive adults.  The Youth Services Center has been created as part of the Kentucky Education Reform Act (KERA).  The intent of the center is to enhance students' abilities to succeed in school by assisting children, youth, and families in meeting some of their basic needs.  This is done by providing direct services and information and by linking families to agencies in their communities.

Who may use a Youth Services Center?

All children and youth (and their families) enrolled in Meyzeek Middle School may use the Center's services, regardless of family income.  Services are especially beneficial for those youngsters who are at risk of not performing well in school.

What services does the Meyzeek Youth Services Center provide?

  • Health services and/or referral to health services.
  • Referral to social services.
  • Employment counseling, training, and placement for youth.
  • Summer and part-time job development for youth.
  • Substance-abuse services or referral to substance-abuse services.
  • Family crisis and mental health services or referral to mental health services.
Who runs the Youth Services Center?

The Center is run by a full-time Coordinator who oversees the operations of the Center.  In addition, the Youth Services Center has an Advisory Council comprised of school personnel, parents, and community representatives.
