Forms The new JCPS enrollment forms have been revised and are available on the JCPS website . The new form is two-sided and is not in duplicate. Parents can complete the form online but must must print out a hard copy to sign and return to the school. This will help if you have more than one student. The social security number section has been deleted off the enrollment form as well. |
