OSHA Employee Injury/Illness Reporting

Each facility is required to complete an annual OSHA Recordkeeping report to document work-related injuries and illnesses. Listed below are required forms and a brief description of use:

  • OSHA Form 301—Injury and Illness Incident Report—When a recordable case occurs, information must be documented on this form.
  • OSHA 300—Log of Work-Related Injuries and Illnesses—You must record information about every work-related injury or illness that involves loss of consciousness, restricted work activities, days away from work, medical treatment beyond first aid, or injuries or illnesses that meet applicable guidelines.
  • OSHA Form 300A—Summary of Work-Related Injuries and Illnesses—This form must be completed and posted from February 1 to April 30 for OSHA recordable cases occurring during the previous calendar year.
